United healthcare credentialing trips people up for a structural reason. UnitedHealth Group owns two things: the insurer, UnitedHealthcare, and a large services business called Optum. Several networks patients think of as "United" are actually credentialed, contracted and managed by an Optum company.
Apply to the wrong one and nothing happens. Nobody forwards it. This guide explains which door to knock on; for the work itself see our medical credentialing services.

Which Optum entity owns a product varies by state, and by product line within the same carrier. Confirm it for your market rather than assuming.
Optum behavioral health credentialing
Optum credentialing behavioral health applications are the first filing worth making for most therapists.
Optum behavioral health credentialing covers the largest behavioral network in the country. For most therapists it is the first application worth filing.
- Optum pulls credentials from CAQH. The profile has to be complete, attested, and open for Optum to read.
- Applications go through Optum's own provider portal rather than UnitedHealthcare's. If you are hunting for the Optum individual therapist credentialing form, it is issued from there once your request is accepted. It is not a public download.
- License type matters. Independently licensed clinicians, LCSW, LPC/LMHC, LMFT, psychologists, PMHNPs, are credentialed individually. Pre-licensed associates generally are not.
- Group versus individual. A group practice needs a type 2 NPI and a group contract; each clinician is then credentialed and linked to it.
- Panel status varies sharply by county and license type. Optum closes panels where it considers the network adequate, and reopens them on network-need arguments, evening availability, a second language, a specialist modality, or telehealth coverage of an underserved county.

Optum Physical Health
For physical therapy, occupational therapy and chiropractic, Optum Physical Health manages the network for many UnitedHealthcare products and for some other carriers as well. It also applies its own utilization management, visit limits, treatment plan review and medical necessity standards that are typically stricter than the plan's own.

What goes wrong most often
- Applying to UnitedHealthcare when Optum owns the network. The most common single mistake, and the most expensive in lost time.
- CAQH attestation lapsed during the review. It is due every 120 days.
- Optum not granted access to the CAQH profile.
- Medicare Advantage assumed to be included in the commercial contract. It is usually a separate participation election.
- Roster not maintained for group practices, so a new clinician's claims are filed before their link is effective.
